Service Description

VNA Extended Home Care, an affiliate of the Visiting Nurse Association of Indiana County, provides a broad range of in-home care and services for clients of any age throughout Indiana County. The emphasis is on personal care assistance and home management in order for individuals in need of assistance-especially the elderly and disabled-to maintain an independent lifestyle. Private duty nursing is also available.

Personal Care Assistance - provided by certified nursing assistants. May include help with bathing, grooming, feeding, dressing and other activities of daily living

Companion - provides companionship and performs tasks which maintain client safety; may provide standby assistance with personal hygiene activities

Homemaker/Housekeeper - includes light housekeeping tasks such as laundry, vacuuming, meal preparation and other services as needed by the client such as grocery shopping, meal planning and errands

In-Home Respite Care - enables the caregiver to leave the home for brief periods of time without leaving the ill or disabled family member unattended

Infant/New Mother Care - provides personal care services to newborns and new mothers; housekeeping services are also available

Care Management - skills services are provided by a registered nurse; services include a physical/functional assessment and home and safety evaluation with care planning and community referrals as necessary

Geriatric Care Management- aimed at helping people maintain an independent lifestyle at home. The system was created to help patients manage their ongoing care.The program identifies and refers clients to services such as long-term health care as well as residential, financial, emotional and social services.

Mom and Baby Club- Help with basic baby care, meal planning, cleaning, laundry, breast-feeding, shopping and errands are some of the services offered. Services are not typically covered by insurance. Mothers may be self-referred, or services may be purchased as a gift, such as for a baby shower.

Nuring Care provided by an RN or LPN. Service is available 24 hours as needed.
